Responsibilities of the Licensed Practical Nurse (LPN):

• Perform assessments in people's homes within your local area, each typically taking around twenty to thirty minutes.
• Utilize evidence-based practices, including point-of-care testing, and identify diagnoses for care management.

Qualifications Required of the Licensed Practical Nurse (LPN):

• Valid unencumbered LPN license.
• Minimum of 3 years of patient care experience (preferred in primary care/adult/geriatric settings and EMR skills).
• Strong empathy, compassion, and friendliness.
• Exceptional written and oral communication skills.
• Available for at least 18 hours per week.
