Are you a Nurse Practitioner looking for meaningful, supplemental work that makes a difference in patient care? We’re seeking motivated Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare Advantage members in Kent County, Michigan.
This is a guaranteed-pay 1099 contract with a fast turnaround and fixed schedule — ideal for independent providers who want stability and autonomy without the demands of prescribing or follow-up care.
Contract Type: 1099 Independent Contractor
Compensation: $18,600/month (paid $9,300 semi-monthly)
Contract Term: 14 weeks
Start Date: October 1 (partial week), ends December 29 (partial week)
Schedule: Full-time, 40 hours/week (8-hour days)
Fixed schedule options: Mon–Fri, Tue–Sat, or Wed–Sun
No time off permitted during the contract term
Holiday Closures: November 27–30, December 25–28
Primary Coverage: Kent County, MI
Providers must reside in Kent County or an immediately surrounding county
Open to providers outside Kent/neighbors, but no hotel or rental cars provided
Mileage reimbursement: begins from first patient visit of the day (not from home → first visit, or last visit → home)

Malpractice coverage + collaborating physician provided
Company-issued iPad with AthenaOne documentation system
Mileage reimbursement (as outlined above)
Conduct in-home Health Risk Assessments for Medicare Advantage members
Review medical history, medications, lifestyle, and social determinants of health
Perform basic physical and mental health evaluations
Provide preventive care education
No prescribing, no on-call, no follow-up care
Active Nurse Practitioner license in Michigan (all specialties except PMHNP)
Availability to work 40 hours/week for the full contract term
Willingness to travel within Kent County (and surrounding areas if required)
Strong interpersonal skills and a patient-centered approach
